Registration Form 2011 – 2012
Early Bird
Registration: Save $ 25 per student if you register by June 30, 2011. A late registration
fee of $ 25 per student will be charged for all registrations received after September
1, 2011. Please fill out one registration form per family, and complete one
student placement form per student. Send all of this along with your payment to
the mailing address listed at the bottom of the form.
